CCC Resource Network

Trusted starting points for common aging and caregiving needs

Cooper Care & Concierge shares community resources to help families explore their options. Inclusion does not guarantee availability or suitability for every individual. Families are encouraged to evaluate providers based on their own needs and preferences.

🧡 Care Coordination & Advocacy

When care involves multiple providers, appointments, services, family members and decisions, I help bring it all together.

Cooper Care & Concierge identifies what needs attention, connects the right resources, coordinates communication and follow-through, and helps ensure your loved one’s needs and wishes stay at the center of the plan.

I don’t just give you a list of resources—I can help execute the plan, oversee the moving pieces, and follow through so things get done.

The result? Less time spent making calls, tracking details and figuring out what comes next—and more time for families to simply be family.
