Community Connections: Collaborative Care Network Supporting Families Through Trusted Partnerships
Our role is to coordinate the many moving pieces so nothing falls through the cracks.
At Cooper Care & Concierge, we believe aging support works best when experienced professionals collaborate.
Families often need more than one type of service at different points in the aging journey. Our role is to help coordinate care, reduce confusion, and connect families with trusted professionals when additional support is needed.
Over time we have built relationships with professionals across North Texas who share a commitment to compassionate, ethical, and coordinated care.

Cooper Care & Concierge at the Flower Mound Senior Center Health Fair
Cooper Care & Concierge is pleased to participate in the Encompass Health Health Fair at the Flower Mound Senior Center on Friday, September 11, from 9:00 a.m. to Noon.
This community event brings together health, wellness, aging, and caregiver resources for seniors and the families who support them.
At the Cooper Care & Concierge table, visitors can learn more about care coordination and advocacy, aging-in-place planning, caregiver respite and support, VA and community resources, hospital-to-home transitions, long-distance caregiving, coordinating multiple healthcare providers, and preparing for changing care needs.
Not sure what you need? That's okay.
Sometimes the first step is simply talking through what's happening and asking the questions that have been sitting in the back of your mind:
Can Mom continue living safely at home? Who coordinates everything when several doctors are involved? What happens if the primary caregiver needs a break? What should we put in place now so we're not scrambling later?
Cooper Care & Concierge helps seniors and families understand their options, connect with appropriate resources, and coordinate the many moving pieces involved in aging and care.
You don't have to wait for a crisis. You just need a place to begin.
Event Details: Friday, September 11 | 9:00 a.m.–Noon | Flower Mound Senior Center, 2701 West Windsor Drive, Flower Mound.
Come meet Melodye McKaye, Owner and Care Advocate with Cooper Care & Concierge, and bring your questions.
